What is heavy menstrual bleeding?
Heavy menstrual bleeding (HMB) is common, with around 1 in 3 people who menstruate seeking care for it at some point (1,2). It can affect quality of life, including daily activities, sleep, energy levels, and productivity, and may be linked to an underlying health condition (1,2).
In the past, HMB was defined as the loss of more than 80mL of blood per cycle. However, this definition is rarely used in practice today, as measuring blood loss is difficult and doesn’t reflect how people actually experience their periods (1,3).
Instead, HMB is now defined more practically: if your bleeding feels heavy and disrupts your daily life, it is considered heavy menstrual bleeding.
Signs of heavy menstrual bleeding can include (1,4):
Periods that last more than 8 days
Bleeding that soaks through a pad or tampon every hour for several hours in a row
Needing to use more than one menstrual product at a time
Waking up to change menstrual products at night
Passing large blood clots (about the size of a coin or larger)

HMB is not a condition itself—it’s a symptom that can have different underlying causes. Understanding what’s causing the bleeding is an important step in choosing the right treatment (1,5).
Healthcare providers typically group causes into two categories:
Structural causes
These are physical changes in the uterus that can often be seen on imaging (like ultrasound), including (1,2,5):
Fibroids: Non-cancerous (benign) growths in the muscle of the uterus
Polyps: Small growths in the uterine lining
Adenomyosis: When tissue similar to the lining grows into the muscle of the uterus
Malignancy: While less common, it’s important to rule out precancer or cancer, especially if bleeding patterns are new or changing
Non-structural causes
These are not visible on imaging and can include (1):
Ovulatory dysfunction: When ovulation does not occur regularly, which can lead to heavier or unpredictable bleeding
Bleeding disorders: Conditions that affect blood clotting can increase menstrual blood loss
Endometrial issues: The uterine lining itself may contribute to abnormal bleeding, even when cycles appear regular
Medications: Certain medications, like blood thinners or some IUDs, can increase menstrual flow
Sometimes, no clear cause is found.

How can heavy bleeding affect your body?
Heavy menstrual bleeding can go beyond your period and impact your overall health. It may make daily activities more difficult and is often linked to symptoms like lower abdominal pain, fatigue, low energy, or shortness of breath (4).
Iron deficiency and anemia
Ongoing blood loss can reduce your body’s iron stores and lead to iron deficiency or iron deficiency anemia (6). This is one of the most common complications of heavy menstrual bleeding and an important reason why symptoms like fatigue and shortness of breath can develop (6).

In some cases, a hysteroscopy may be performed, where a thin tube with a small camera is used to look inside the uterus. An endometrial biopsy may also be done, which involves taking a small sample of the uterine lining to examine it under a microscope (1).
Heavy bleeding and period pain
Heavy bleeding and period pain (also known as dysmenorrhea) can occur together, but they don’t always have the same cause (8). Having period pain can reduce quality of life and can lead to a higher risk of depression and anxiety (8).
Pain may be:
Primary dysmenorrhea: Caused by prostaglandins, which are chemicals made in the uterine lining that trigger uterine muscle contractions (9). Levels are highest at the start of a period, which is why pain is often strongest then and improves as bleeding continues (9).
Secondary dysmenorrhea: Linked to underlying conditions like fibroids, endometriosis, or adenomyosis, and tends to worsen over time, last longer, and may start before or continue after bleeding (9,10).
Some conditions can cause both heavy bleeding and pain, which is why these symptoms are often connected (8-10).
Treatment options for HMB
There are different ways to treat heavy menstrual bleeding, and the best option depends on what’s causing it and how it affects your daily life. Factors like your age, overall health, medical history, and personal preferences also play a role (4).
Some people might want to stop their periods, while others prefer a predictable cycle or lighter bleeding. For some, fertility is important, while others may focus on reducing pain. Discussing options with a healthcare provider is key (4).
Medical and non-surgical options
These are often the first step. If there are no signs of an underlying structural cause, you may be able to start treatment without further testing (11).
Hormonal options

These can help reduce bleeding, regulate your cycle, or stop periods altogether (2,4,11). The IUD works directly in the uterus to thin the uterine lining and may be more effective than other hormonal options (12,13).
Non-hormonal options
Tranexamic acid: Reduces bleeding
NSAIDs (like ibuprofen): Help with both bleeding and pain (4)
Other options may include iron supplements (if anemia is present) or medications for specific conditions (4).
If your symptoms are severe or don’t improve, further evaluation and other treatment options may be considered (11).
Supportive technology for symptom management
Beyond traditional medical treatments, innovative technology offers additional ways to relieve the daily impact of cycle-related symptoms.
Non-invasive neurostimulation
Some forms of neurostimulation—specifically non-invasive methods—are being studied as a way to help manage symptoms like pain and fatigue. These approaches don’t treat the underlying cause of heavy bleeding, but may support how your body responds to symptoms while you and your healthcare provider address what’s driving the bleeding.
One method, transcutaneous auricular neurostimulation (tAN®), uses a gentle electrical stimulation applied to the ear to activate vagus and trigeminal nerve pathways involved in pain, inflammation, and autonomic regulation (14). OhmBody™ is currently the only device with this technology.
Two small open-label pilot studies suggest that using tAN® during menstruation may help reduce menstrual blood loss and shorten periods in people with heavy menstrual bleeding (14,15).
Participants experienced reductions in menstrual blood loss of around 50% and periods that were about 20% shorter than baseline (14,15). They also reported improvements in menstrual symptoms, including cramping pain, fatigue, and health-related quality of life (14,15).
The follow-up study, which evaluated an updated electrode design, also found high levels of comfort and satisfaction with the device (15).
The exact mechanism is still being studied, but findings suggest that neurostimulation may help modulate nerve pathways and support the body’s regulation of bleeding and symptoms (14,15).
Transcutaneous electrical nerve stimulation (TENS)
Another supportive non-invasive method is transcutaneous electrical nerve stimulation (TENS). This involves using a small, battery-powered device to send mild electrical impulses through the skin to stimulate nerves. TENS may help reduce period pain for some people and can improve quality of life, sometimes reducing the need for pain medication (16).
While results are promising, more research is needed to better understand its effectiveness and how best to use it (16). There is currently limited evidence that TENS reduces menstrual bleeding itself.
Because heavy menstrual bleeding can have different underlying causes, it’s important to discuss treatment and support options with a healthcare provider, who can help tailor a plan based on your individual needs (4). These approaches are still being studied, but they may offer additional support alongside medical treatment (14).
Surgical options
Surgery may be considered if other treatments don’t help or if symptoms are severe:
Endometrial ablation: This destroys the lining of the uterus to reduce or stop bleeding (6). It’s typically considered after other treatments haven’t worked and is not recommended if pregnancy is still desired (2).
Myomectomy: This removes fibroids while preserving the uterus. Depending on where the fibroids are located, this can sometimes be done with a hysteroscopic approach, a less invasive procedure performed through the vagina and cervix to remove fibroids inside the uterine cavity (2).
Hysterectomy: This involves the removal of the uterus, which stops periods permanently and makes pregnancy no longer possible. It’s usually considered when other treatments have failed or are not suitable (2,3,17).
Other procedures may be used in specific situations. Uterine artery embolization can reduce abnormal uterine bleeding caused by fibroids by cutting off their blood supply (17). Dilation and curettage may be used for short-term control of acute bleeding or for diagnostic purposes (17).
Long-term health and support
Heavy menstrual bleeding can affect more than just your period (10). It can have longer-term effects on physical and mental health, which is why ongoing support and follow-up care matter.
Heavy menstrual bleeding is closely linked to iron deficiency and anemia. Even when iron supplements are used, both can still be common. Screening for heavy menstrual bleeding, iron deficiency, and anemia can help identify those at risk and guide care (18).

Research shows that heavy bleeding and irregular cycles may be linked to higher levels of anxiety and depression (10,19). For some, this can mean feeling more stressed, overwhelmed, or avoiding activities during their period (10,19).

Diagnosis of HMB is based on your symptoms, medical history, and tests like blood work or imaging. Sometimes procedures are needed to look inside the uterus (1).
