Navigating your period during a heat wave

An expert Q&A with Clue's Science Team

If you've ever noticed that your period feels different when the temperature rises, you're not imagining it. Summer heat can be hard enough on its own, but for many people with cycles, symptoms like bloating, fatigue, irritability, and headaches can feel even worse.

Extreme heat and your menstrual cycle can make each other worse in ways that scientists are only beginning to understand fully. To find out why this might be, we spoke with Clue's Science Team and Chief Medical Officer, Dr. Charis Chambers.

They explain what's happening inside your body during extreme heat and share ways you can protect your sleep, mood, and overall well-being until cooler days return.

1. How does hot weather make period symptoms like fatigue, irritability, and headaches worse?

During a heat wave, your body is working overtime to regulate your temperature and keep you as cool as possible. 

Menstruation makes this process even harder by adding extra strain to your body’s hormone, blood flow, brain chemistry, and fluid balance systems. This can lead to experiences such as bloating, exhaustion, irritability, and headaches.

Heat exposure also directly disturbs sex hormone balance. A study of nearly 20,000 people found that chronic exposure to extreme heat significantly decreased estradiol, progesterone, and testosterone levels.

These changes can disrupt the finely tuned brain-to-ovary communication system. Since progesterone and estrogen influence mood, fluid balance, pain perception, and energy levels, heat-induced hormonal changes can intensify premenstrual and menstrual symptoms that are already driven by normal hormone fluctuations that happen during your cycle.

During the luteal phase, progesterone levels rise. Progesterone also raises both core body temperature and skin temperature. It also delays the point at which sweating begins.

This means that someone in their luteal phase may start a heat wave with higher body temperature and begin sweating later, making it harder for the body to cool itself. This increases the risk of heat exhaustion and associated symptoms such as fatigue, weakness, dizziness, and headache.

Studies of women in high-temperature environments also show higher levels of histamine and glutamine, and lower levels of important serotonin byproducts. 

This suggests that heat stress may change the balance of brain chemicals in ways that promote irritability, anxiety, and mood changes. Heat can also disrupt sleep, which may increase fatigue, irritability, and sensitivity to pain.

2. Why do bloating and body aches feel more severe in the summer heat?

Menstruation is already an inflammatory process, meaning the body naturally produces chemicals that help shed the uterine lining.

Right before your period starts, estrogen levels drop, reducing some of the hormone's anti-inflammatory effects. 

At the same time, hot weather also triggers an inflammatory response throughout the body. Together, these effects can increase inflammation, making cramps, body aches, and other period symptoms feel more intense.

Heat can also make bloating feel worse, but not necessarily because you're retaining more water. Research suggests that PMS-related bloating is mainly caused by fluid moving into body tissues rather than simply holding onto extra water. 

During the week before your period (the luteal phase), hormonal changes make blood vessels leak more fluid into surrounding tissues, which contributes to bloating. 

Progesterone and estrogen also affect how your body manages fluids. Studies show that if you're drinking enough water, your overall hydration stays about the same throughout your menstrual cycle, even in the heat. 

This suggests that summer heat may make bloating feel worse because of changes in how fluid is distributed throughout the body, rather than because you're dehydrated.

3. How do changes in estrogen and progesterone influence the body's ability to regulate temperature? Are there certain phases of the menstrual cycle when people may be more vulnerable to heat-related discomfort?

Estrogen and progesterone have opposing effects on temperature regulation:

  • Estrogen lowers your body temperature by helping you release heat more easily.

  • Progesterone raises your body temperature by making it harder for your body to sweat and cool down.

Progesterone peaks during the luteal phase. Because of this, it is harder for your body to manage heat during this part of your cycle.

4. Why might people living with conditions such as PMDD, endometriosis, menstrual migraines, autoimmune conditions, or hormonal sensitivities find heat waves particularly difficult to manage?

People with PMDD, endometriosis, menstrual migraines, autoimmune conditions, and hormonal sensitivities face extra vulnerabilities during heat waves. 

This is because heat stress can affect the same inflammatory, brain chemistry, blood flow, and hormonal pathways that are already disrupted in these conditions.

PMDD

PMDD is characterized by an increased sensitivity to normal changes in estrogen and progesterone levels. These hormone shifts can affect brain chemicals like serotonin and GABA, which play a role in mood and emotional regulation.

Heat stress changes how your brain processes serotonin and boosts histamine levels. This brain-chemical shift makes you much more likely to feel irritable and anxious.

Endometriosis

Endometriosis is a long-term condition fueled by estrogen that causes ongoing inflammation. Endometriosis tissue creates a constant state of inflammation, and heat stress triggers many of those same inflammatory pathways

Menstrual migraines

Menstrual migraines are mainly triggered by falling estrogen levels before the period and inflammation during the period. Heat adds extra triggers on top of this—like dehydration from sweating, reduced blood flow to the brain, and forcing the brain to work harder to stay cool.

5. Are there any symptoms that women with these conditions should pay especially close attention to during heat waves?

Women with conditions such as PMDD or endometriosis should watch for two types of symptoms:

  • Signs that their existing condition is getting worse, possibly because of the heat

  • General signs of heat-related illness

One challenge is that early symptoms of heat exhaustion (tiredness, headaches, nausea, dizziness, and muscle cramps) can look very similar to the symptoms they already experience from their condition. Because of this overlap, it can be harder to tell when heat is causing a problem, which may delay recognizing and treating heat-related illness.

For people with PMDD, the biggest concern during a heat wave is worsening mental health. Research shows that extremely hot days are linked to an 8% increase in emergency room visits for mental health conditions, including mood disorders, anxiety, and self-harm. 

People with PMDD who experience suicidal thoughts, hopelessness, or feel like they are losing control during the luteal phase should speak to a healthcare provider. PMDD is linked to a higher risk of suicidal thoughts and behaviors.

If you have thoughts of harming yourself or feel that you may act on them, seek immediate help by contacting your local emergency services or a crisis hotline, or go to your nearest emergency department. 

In terms of endometriosis, symptom flares are common and highly variable. Severe abdominal pain with signs of bowel obstruction (vomiting, distension, inability to pass gas) requires emergency evaluation regardless of whether the person has endometriosis or not. 

For menstrual migraines, temperature is linked to migraine attacks. Red flags that should prompt urgent evaluation include:

  • A headache that is considerably different from your typical migraine pattern

  • A migraine with new symptoms, such as vision changes, numbness, or trouble speaking, that you haven't experienced before

  • Or migraine accompanied by fever and neck stiffness

6. What are the most effective, evidence-backed strategies to manage hormonal symptoms during heat waves?

The best ways to manage heat include cooling your body, staying hydrated, adjusting your activity to your cycle, protecting your sleep, and reviewing your medications with your healthcare provider if needed.

Active body cooling means cooling the person and not just the environment.

This can be done with:

  • Cool showers

  • Ice towels, especially applied to the neck and chest area

  • Drinking cold water

  • Wearing something like an ice vest

When temperatures rise above 99°F (37°C), fans may not help you cool down. In some cases, they can even increase dehydration if sweat isn't able to evaporate effectively. Staying hydrated is also important.

It’s also important to check your medications with a doctor, as some can change how your body handles heat, for example:

  • SSRIs and anticholinergics can interfere with temperature control

  • Beta-blockers make it harder for your heart to adjust to extreme heat

  • GnRH medications lower estrogen, which can make you more sensitive to high temperatures

7. How can daily habits make a difference to how we feel in hot weather?

Sleep disruption during heat waves has a major—yet often overlooked—impact on PMS and PMDD. Those with PMDD often experience worse insomnia, trouble concentrating, and fatigue during the luteal phase. 

Poor sleep is also linked to a higher risk of migraine. To stay cooler at night, try cooling your head, neck, and back, taking a cool shower before bed, or using a cooling mattress topper. 

Exercise is also important. During heat waves, try not to stop exercising completely, as regular physical activity can help reduce PMS symptoms. Instead, shift exercise to early in the morning or later in the evening when it’s cooler. 

You may also want to reduce the intensity of your workouts during the luteal phase and choose lower-intensity activities such as yoga, which produces less body heat than aerobic exercises.
